You can contact the Sunderland Family Support Team via email sunderlandfamilysupport@wearedaisychain.org or if you are a parent and would like to register with our service, please fill in the form below. Please note the Family Support Service is self-referral only'. If you are a professional, please guide the family to register either online via the form below, by telephone or emailing the team. The team will get back to you as soon as possible, please allow 48 working hours for a response.
